Telephone 01651 843910 (answerphone) The role of the District Nursing Team is to carry out nursing care to patients who are housebound and unable to get to the surgery. Along with the provision of nursing care they also provide support and information to the patient, their carer and the family. The community nursing team work in partnership with the patient and co-ordinate a network of care and support, work in collaboration with GPs, Marie Curie Nursing Service, Social Services and other Health Care Professionals to deliver person centred care. Their main focus is wound management, continence management, long term conditions, care of the terminally ill and palliative care. |
